* This package does NOT include cash advance items, such as Indiana sales tax, cemetery expenses, newspaper notices, certified death certificate copies, flowers/florist, clergy honorarium, musician(s), hairdresser, etc.

These items are the responsibility of the purchaser(s) at the time. 

Payment of funeral home charges due at time of arrangement with cash or check, or by insurance assignment. A fee of up to 5% will be added for amount assigned to the funeral home.
